The guidance changed. In the CDC 2021 update doxycycline for a week became the recommended regimen for chlamydia, with the single dose azithromycin moved to alternative.

The main driver was rectal infection, where the week long course performs noticeably better. Since rectal infection is often present without anyone knowing, treating everyone with the more effective option was the simpler call.
