Worth separating test of cure from the three month retest. The first asks whether the treatment worked. The second asks whether you have been reinfected. They are different questions and both exist.

Ask about testing as well as treatment. Testing in men is less reliable, which is partly why contacts are treated on the basis of exposure rather than waiting for a result.

Worth checking which test he had and whether it was type specific. A test that does not distinguish HSV-1 from HSV-2 is not answering the question you think it is.

Mycoplasma genitalium is the one that surprises people. Several guidelines specifically advise against screening for it without symptoms, because treating it unnecessarily drives resistance, and it is already a difficult organism to treat.

The distinction worth holding onto: a licensed product with trial data behind it is a different category from a shop bought extract, even when the plant is the same.

Reinfection, not treatment failure. Those are two different questions and the two tests answer one each.

The rate of testing positive again within a few months is high enough that it is worth a routine appointment, usually because a partner was untreated or a new partner came along.

If adherence has slipped, say so now rather than at the next appointment. That is the one explanation that does change what happens next.
